What to Expect

Few people like to go to the doctor, therefore we strive to make your visit with us as comfortable and pleasant as possible. We believe strongly that our patient’s comfort is an important part of quality dental care. During your first visit, the doctors and staff will complete an in-depth medical and dental health history. They will gather all necessary diagnostic information to accurately evaluate you for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease.

Note: Treatment is not traditionally rendered at the initial appointment. The consultation is intended to gain information, listen to your questions and concerns and to help provide solutions leading you to better health.

We recognize that your time is valuable, in order to assist us and expedite your initial visit to the office please provide the following information:

  • Medical History – Complete Medical History prior to appointment. Please click on the button below to download our patient registration and medical history form.
  • Radiographs (x-rays) – We will need current Radiographs (x-rays) or CT Scans in order to accurately make a diagnosis. If your referring dentist has taken radiographs, you may request that they be forwarded to us. If you have not been to the doctor for extended periods (greater than 2 years) or if the radiographs are unavailable, Dr. Lupovici will take any necessary radiographs.

Please Note: Please notify the office if you have a medical condition or concern prior to your consultation (e.g. artificial heart valves or joints, severe diabetes, hypertension or other systemic condition that would require you to be pre-medicated).

All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
